What is the difference between first name, last name, middle name and surname.?

I am totally confused while filling out forms like driving, bank, passport application forms.


My Name is Nagaraj and my father's name is Chellappan.





How do i fill up my name detail when so many names are asked for.


In my understanding my first name should be Nagaraj.

What is the difference between first name, last name, middle name and surname.?
Last name is father's family name. John David Smith: Smith is Father's name. Father was, say Charles Smith. In business relationships, he would be called Mr. Smith. His wife would call him Charles.





First name is given name, what your Mother or sister may call you (NOT a nickname, tho). John David Smith: First name John.





Middle name is second given name, if any. John David Smith: David is middle name. Your Mom might use it when she is REALLY angry at you: JOHN DAVID, you get over here to me, RIGHT NOW! lol





Ordinarily not used.





I was raised Roman Catholic, and have FOUR names! First name. Middle name. Confirmation Name. Last name. :-))





Usually middle names are not used, unless necessary to differentiate between two people with same name: John D. (for David) Smith vs John H. (for Henry) Smith, if both in the same class in school, or on the same list.

Where are you from? Do people use two names in your culture? I know in some, it is not done.
Reply:Last name or surname or family name means the same thing.

Reply:Nagaraj would be your first name then, the way it sounds. If you have name after that, and then your last name, Your first name would be your first, after that is your middle name, and then your last name.





Surname is just if you are a decendant, basically if you are named the same as your father or grandfather. So if your name was Chellappan and then your family's last name, then you would have the surname II, which means "the second", meaning you are the second one born with that same name.

Reply:Last name is your family name. First name is the one that you answer to. Nagaraj would be your FIRST name and Chellappan would be your LAST. A middle name is just a second first name. It is optional. Surname is the same (as far as I know) as last name.
Reply:first name is self explanatory, its your first name..e.g. Greg, Kylie, Marsha, Annabella, Alexis...middle name is your middle name you have if you have 2 or 3 put them down. last name and surname i think is the same thing. which is the current last name you have..let it be if your married single the only last name/s you have at that point in time.
